To purchase this personalization service you must buy the document that you need and additionally purchase this personalization service with that order for each document that you want personalized (this personalization service may also be purchased alone after the initial order(s) if necessary or desired).
Then, you must either scan and email to us the first two pages, and signature page, of the letter that you received from the IRS, OR fax it to us at (202) 280-1301.
We cannot provide this personalization service unless and until we receive from you the letter that you received from the IRS. If you fax the IRS letter to us ((202) 280-1301), rather than scan and e-mail, be sure to include an e-mail address on the Fax that we can use to email the finished document back to you after we have personalized it.
If you do not have an e-mail, get one, the mail is too slow for us to rely on for timely deliveries to you.
